.com is a place where you can start a blog without having web hosting
.org is software to start your own blog on your website
Both you can change themes but only using .org software on your website will allow you to customize it fully.

You have full control of your blog's setup and code when you use .org or have a web host install Wordpress for you instead of using .com, which has a fixed set of very few options. You can do adsense, custom themes, and everything. I highly recommend bluehost, they make it really easy to set up the blog so you don't have to download files from wordpress.org and do the install yourself.
